The PLAN Institute is a global leader in creating personal support networks: networks of caring that combat social isolation, support belonging and enable the contributions essential to a meaningful life. PLAN has worked for twenty years to develop personal networks for people with disabilities, seniors and others.

In 2007, PLAN launched Tyze, an online community tool for personal support networks. It features calendaring, task management, profiles for network members, and most crucially, storytelling.
